Please make sure the School Nurse has the following forms on file:
Florida Immunization Certification
(may be obtained by pediatrician, family doctor, or Florida Health Department) ~ Returning students should already have this form on file. Rising 7th graders and new students need to bring in this form by the first day of school.
*7th Grade Requirements: All 7th grade students must have their 7th grade vaccinations prior to school entry (tetanus, diphtheria, and pertussis). In accordance with the Florida Health Department regulations, proof of immunization is required prior to beginning the 7th grade school year. Please bring the blue, updated Florida Immunization Certification form to the Upper School Nurse, as well as the annual Out-of-Door health forms listed below.
Florida Physical
(may be obtained by pediatrician, family doctor, or Florida Health Department) ~ This form is specifically for a sports physical, but a standard Florida physical form from your family doctor will also be accepted. Only if your child plays a sport is a new physical required every year.

If your child has any specific health conditions such as allergies, asthma, diabetes, or seizures, please fill out the appropriate form below:

Prescription Medication
If your child needs to receive prescription medication during school hours, please note that this form will need to be filled out by both parent and physician and returned to the School Nurse. All prescription medication must be brought in by the parent/guardian, given to and signed in officially by the School Nurse; this is a Florida Health Department regulation and allows the School Nurse to legally administer prescription medications to your child. Prescription medications cannot be transported to or from school.

If your child is going to play a sport during the school year, please fill out this additional paperwork, required annually by the Athletic Department before participation is allowed.
